Steps to Avail Letter of Authorization (LOA) for Out-Patient Diagnostic (OPD)

  1. Submit an LOA Request
    -Fill out the LOA request form completely and accurately to avoid delays in processing. 

  2. Receive and Print the LOA
    The LOA will be sent directly to the email address you provided.
    -Download and print the LOA in PDF format to ensure optimal quality and acceptance.

  3. Proceed to an Accredited Medical Facility
    Visit your chosen accredited hospital or diagnostic laboratory for your laboratory test or procedure.

    Mindanao Medical Facilities

  4. File PhilHealth Requirements (If Applicable)
    For procedures requiring PhilHealth coverage, ensure you file the necessary PhilHealth documents before your test or procedure.

Important Reminders:

  • The Life and Health Letter of Authorization (LOA) is valid for 7 days from the date of issuance. Ensure it is used within this period.

  • The LOA is automatically generated by our system and does not require a signature from the sender.

  • Fill out the LOA request form correctly, following all instructions to prevent processing delays.

  • A doctor’s request with a diagnosis must be attached when requesting an LOA.

  • If you do not have a doctor’s request, please schedule a consultation first before requesting an LOA.

  • You may also obtain an LOA in person at any of our Life and Health Branches. Refer to the Life and Health Branch Directory.
