Here’s how you can access your membership benefits and obtain the necessary forms

Step 1

  • If treated in a non-accredited facility or by non-accredited doctors, you may file for reimbursement, subject to Life & Health evaluation.

Step 2

  • Fill out the reimbursement form and attach required documents to it.
  • Ensure all attached invoices or official receipts are original copies.

Step 3

  • Submit the form and the required documents to your company Nurse/HR or visit the nearest Life & Health office.
  • The reimbursement request will be processed within 30 working days upon receipt of all required documents at our office.
  • Life & Health reserves the right to impose additional requirements to justify payment of claims or deny even upon completion of requirements. Reimbursement is subject to plan coverage only.
